CASE MANAGERS

As a case manager, you can be assured you are working with a dedicated and experienced Speech and Language Therapist.

Currently, I am working with several Case Management services to provide the best client centered care within a holistic approach. 

​

I provide clear, upfront costs and quotes for assessment and treatment from the outset upon request.  

​

After instruction, I carryout a thorough initial assessment considering the client, their family and the team around them. 

​

I then write a comprehensive initial assessment report detailing findings focusing on the client's communication needs, dysphagia needs or both.   

​

My management of the client thereafter will be provided based on their needs. Therapy sessions will be bespoke to enable the best results possible following individualised outcomes which are Specific, Measurable, Achievable, Realistic and Timely (SMART).

To enable effective practise, I liaise regularly with the client's Multi-Disciplinary Team.
